Background information about the fund
Awareness Raising and Prevention
- Raise awareness of the important role blood pressure plays in our health.
- Help communities better understand risks, signs and symptoms of high blood pressure.
- To improve access to blood pressure reading equipment locally and normalise regular BP reading.
- Help communities to ‘know their numbers’.
Making Every Contact Count
- BP checks create a natural opportunity to start conversations about health and lifestyle.
- Staff can signpost individuals to local support services and resources
- Trust and familiarity with community organisations encourages people to engage with their health
- Social prescribing can encourage conversations around further support regarding the wider determinants of health
Improving Healthcare Outcomes
- Early detection of hypertension reduces the risk of stroke, heart attack, and kidney disease.
- Increased awareness and self-management of health conditions leads to better long-term control (e.g. diet, exercise, medication adherence).
- Better engagement with prevention services, including NHS Health Checks and lifestyle programs.
