Home Plus Referrer Survey

The Health and Care Evaluation Service at the West Yorkshire Integrated Care Board is conducting a survey to learn more about Home Plus and how people feel about it.

Do you currently refer people to the Home Plus service?

Please respond to the short survey by Wednesday 19 March.

The Home Plus service comprises of various types of support: falls prevention, hospital discharge, warmth and energy efficiency support, hazard repairs and benefits advice. There is also additional support around heating on prescription, and then separate but linked support available relating to Breathe Easy homes.

Please note: Heating on prescription is not part of the core Home Plus service. Breathe Easy homes is not part of the Home Plus service but is delivered by Care and Repair. However, as part of this evaluation, we are interested in these elements as well as the core elements of the Home Plus service.

Your answers will be collected and held securely as legally required by the General Data Protection Regulations UK (GDPRUK). This exists to give people more control of their personal information and how it is used. The responses you give will only be shared with people directly involved in the work around this survey. Your responses to this survey will be held safely until 31st March 2027. At this point, your responses will be safely destroyed.
