Health services are changing, with a bigger emphasis being placed on care in communities.

Leeds Community Healthcare are partnering with Healthwatch Leeds to find out what healthcare outside of hospitals means to people and how it can be reshaped to suit communities’ needs.

How you can help:

  • Tell us your views in our survey
  • Share the survey link with the people you know/your colleagues
