Community Care: A Third Sector perspective survey

The NHS is looking to make what it calls “Three Shifts” to transform and improve the care it offers.

They are:

  • moving more care from hospitals to communities,
  • making better use of technology in health and care,
  • focusing on preventing sickness, not just treating it.

With that in mind, Forum Central, Healthwatch Leeds and Carers Leeds are working with Leeds Community Healthcare to find out what good community care means to people. Your views as someone working in the voluntary and community sector are a crucial part of this.

We would be very grateful if you could spend ten minutes sharing your thoughts with us by answering the following questions:

  1. Which organisation do you work for?
  2. How ready is your organisation right now to work with the NHS to deliver more care to people in your community? (Consider what currently works well, and what barriers prevent you from working in a joined-up way with NHS colleagues).
  3. Which sections of the NHS do you already work with? E.g. primary care, community health, secondary care.  Is this a formal arrangement that your organisation receives funding for?
  4. What are the main things the NHS could do to support you to help people keep well in your community? (Consider all factors including workforce, IT and estates)
