Have your say on Proactive Care in Leeds
The Health and Care Evaluation Service at the West Yorkshire Integrated Care Board are interested in learning more about the Proactive Care programme in Leeds.
The proactive care programme is about working with people in their own home and local community. It aims to bring staff together from across a range of teams to better support people most at risk of deterioration. The goal is to improve independence, and reduce, delay or avoid demand for services.
To support the programme we have a staff survey that we are looking for people across organisations to complete. This will provide a baseline of how well things are working across Leeds now, and help us shape local development plans for each Local Care Partnership (LCP).
Please could you complete this survey: Proactive Care staff survey – baseline by the end of Friday 18 July and encourage colleagues who also cover the local area to complete it too.
We want to try and capture views from a range of organisations, departments and roles including community groups and all council departments. When it asks which LCPs you link to please tick any that apply to you – we won’t be asking you to answer the survey separately for other LCPs you attend.
