Leeds HomeFirst Programme Shortlisted for National HSJ Award

Leeds HomeFirst Programme has been shortlisted for a prestigious Health Service Journal (HSJ) Award in the ‘Transforming Care for Older People’ category. 

This national recognition shines a spotlight on the incredible collaboration across Leeds to create a person-centred, home-first model of intermediate care. HomeFirst aims to support more people in returning home from the hospital quickly, safely, and with the proper support around them, resulting in independence, recovery, and dignity. 

The shortlisting is a massive credit to the dedication and innovation of partners across the system, from health and social care teams to voluntary sector organisations, who are working together to change lives every day.

This achievement also comes as a part of a broader celebration of Leeds’ health and care system, which has been recognised across several HSJ Award categories:

  • Independent Healthcare Provider of the Year: Healthcare and Leeds Teaching Hospitals – reducing bed days through vital wards
  • Primary and Community Care Innovation: Leeds GP Confederation – Adult ADHD ‘Waiting Well’ Project
  • Workforce Initiative of the Year: Leeds Teaching Hospitals – Creating a sustainable and inclusive NHS Bank workforce

The winners will be announced in November, but whatever the outcome, being shortlisted is an achievement to be proud of.
